Alert: South Korea, Mandatory Q-CODE or Health Declaration Form for Entry into South Korea

Location:  Republic of Korea 

Event: Starting September 1, 2024, the Republic of Korea (ROK) government has mandated that travelers who have visited, stayed, or transited through certain regions, including certain U.S. States (Colorado, Michigan, and Texas), must submit a Q-CODE or health declaration form upon entering South Korea. Please review this complete list of locations that require a Q-CODE or health declaration form when entering Korea.
 

ACTIONS TO TAKE: 

  • When entering Korea, be sure to comply with all immigration requirements, including the Q-CODE or health declaration form. 
  • Ensure your information is updated with the U.S. Embassy at https://step.state.gov .  
  • If you know of any U.S. Citizens that DID NOT receive this message, please advise them   to enroll in the Smart Traveler Enrollment Program (STEP ).  

For more information about the Q-CODE or health declaration form, please contact the Korea Disease Control and Prevention Agency .  For additional information about all ROK immigration rules and regulations, please contact the Korea Immigration Service .  The ROK government also recommends downloading the Emergency Ready App .
